Living Branches Vaccine Clinics

Living Branches will hold Covid vaccine clinics for residents only in April. Residents may register for a Covid vaccine on this page. 

Step 1. Register Online for a Vaccine

Living Branches residents: please register for a vaccine using the links below. Each campus has a separate registration form.

Step 2. Download and Fill Out a Consent Form

Click the orange button below to download the consent form for your Covid-19 booster. You can fill it out on a computer or by printing the form out and writing in your information.

Step 3. Bring Your Insurance Card

In addition to your consent form, please bring your current insurance card to your appointment.
