Flu Vaccination for Team Members
Because influenza presents a substantial health risk to residents and team members, a flu vaccination is required of all Living Branches team members. By Monday, November 3, all team members must receive a flu vaccine at Living Branches or give proof of receiving a flu vaccine elsewhere to your human resources generalist.
Team members requesting a religious or medical exemption should submit a request to chief human resources officer Kristin Thim by Monday, September 15.

Step 2. Download and Fill Out a Consent Form

If you are under 18, a parent or legal guardian must sign your consent form(s).
Step 3. Bring Your Insurance Card
In addition to your consent form, please bring your current insurance card to your appointment.
If you are NOT covered under Living Branches health insurance, please bring your insurance card with you. Team members with Living Branches health insurance do not need to present an insurance card.
