Living Branches will offer the flu and Covid-19 vaccines in September and October. Residents may receive the flu and Covid-19 vaccine, and team members may receive the flu vaccine.
Living Branches Vaccine Clinics
Team Members: Because influenza presents a substantial health risk to residents and team members, a flu vaccination is required of all Living Branches team members. By Monday, November 2, all team members must receive a flu vaccine at Living Branches or give proof of receiving a flu vaccine elsewhere to your human resources generalist. Team members requesting a religious or medical exemption should submit a request to chief human resources officer Kristin Thim by Tuesday, September 15.

Step 2. Download and Fill Out Consent Form(s)
Please download and complete a consent form for each vaccine you will receive. You can fill it out on a computer, or print it out and write on it.
Step 3. Bring Your Insurance Card
In addition to your consent form, please bring your current insurance card to your appointment.
