Living Branches Vaccine Clinics

Living Branches will hold joint flu and Covid vaccine clinics for residents and team members in October. You may receive both a flu and Covid vaccine on the same date or sign up for separate dates to space out your vaccines.

On the day of your appointment, please bring your completed consent form and your insurance card.

Step 1. Register for Your Vaccines

When you register, you will be asked if you would like the flu vaccine, the Covid vaccine, or both. If you would like to get the flu and Covid vaccine on different days, please sign up for two separate slots and indicate which vaccine you would like on each date.

Step 2. Download and Fill Out Your Consent Forms

Click the orange buttons below to download the correct consent forms for your vaccines. You can fill them out on a computer or by printing the form out and writing in your information.

Use the Covid vaccine form if you are getting only the Covid vaccine, or if you are splitting up your Covid and flu vaccines.

Use the flu vaccine form if you are getting only the flu vaccine, or if you are splitting up your flu and Covid vaccines.

Use the combined form only if you are getting your Covid and flu vaccines together during the same appointment.

• Please use the combined form only if you are receiving the flu and Covid shot on the same day and time; otherwise, please complete the Covid and flu consent forms separately.
• Please bring your completed form(s) to your appointment. Extra forms will be available when you check in.
• If you are under 18, your parent or legal guardian must sign your forms.

Step 3. Bring Your Insurance Card

In addition to your consent forms, please bring your current insurance card to your appointment.
