Health Care Admissions Specialist – Full Time

The Health Care Admission Specialist/Clinical Liaison works collaboratively with other team members and is responsible for the comprehensive medical review and screening of all referrals/admission inquiries made to the Living Branches Health Care centers.
Health Care Admissions Specialist Overview:
- Process all inquiries for short term and long term admission to Living Branches Health Care centers (nursing homes)
- Travel to local hospitals to conduct pre-admission screening
- Determine patient’s appropriateness for admission into Health Care consistent with Living Branches policies and admission criteria, in conjunction with the director of Health Care services as needed.
- Assess resident/family needs, consulting with director of Health Care services as needed
- Coordinate with Living Branches billing office for insurance verification as needed
- Track the needs of existing Living Branches residents while they are in local hospitals
- Conduct neighborhood tours for patients and families
- RN/LPN License Required
