Memory support in Health Care

If your loved one has dementia and needs skilled nursing care, they may thrive in a Health Care community that offers memory support. In these areas, those with dementia receive round-the-clock care in a community designed for their needs.

After recent renovations, Country Cottage at Dock Woods is brighter, larger, and better serves those with dementia. A similar community will open at Souderton Mennonite Homes in the summer of 2024.

Living Branches’ Health Care communities are inspected each year by the Center for Medicare & Medicaid Services (CMS), in addition to local and state agencies. Our community consistently earns five stars, indicating top quality, safety, and inspection results.

Our five-star rating isn’t just about safe buildings. It’s primarily a signal of exceptional caregivers. Each member of your loved one’s care team is credentialed, trained well, and up-to-date on the best practices in giving care.

Exceptional staff-to-resident ratio

In our Health Care communities, the staff-to-resident ratio is far higher than state requirements. Your loved one will get to know:

  • Nurses (RNs and LPNs)
  • Certified nursing assistants (CNAs)
  • Physical, speech, occupational, restorative, and creative arts therapists
  • Dietitians
  • Life enrichment coordinators
  • Pastoral care team members

A range of care under one roof

Most care is available on-site, including primary care, podiatry, audiology, wound care, and psychiatry. We also coordinate care with your loved one’s specialists and can provide information they may need to treat your loved one. Transportation to and from specialists is available at an additional cost.

Preserving independence

No two residents are alike, so our team members do not treat them that way. Our team works with each resident as an individual, tailoring care to their preferences. From food and clothing to activities and entertainment, your loved one will direct their own day as much as possible.

Training tomorrow’s caregivers

Living Branches doesn’t just recruit great certified nursing assistants (CNAs) – we also train them. In our dedicated CNA training program, students learn the concepts and methods required by the state. And they do so in a Living Branches community, infusing their training with person-centered care. Throughout their careers, CNAs trained at Living Branches go above and beyond what we think of as traditional nursing home care.

Recover in comfort with a short-term stay

Sometimes a few days at the hospital isn’t enough time to fully recover after an illness or injury. If your loved one needs more time, the hospital may refer you to skilled nursing, which Living Branches provides in Health Care. During a short-term stay in Health Care, our team will guide your loved one along the road to recovery.

Recovery is much easier when therapy is right down the hall – or right in your room. In addition to help with daily activities and medication, your loved one can work with physical, speech, occupational, restorative, and creative arts therapists.

Insurance can be used for short-term rehabilitation. We accept Medicare, Keystone 65/Blue Cross, and UnitedHealthCare for primary insurance coverage. Work with your discharge planner at the hospital to bring your loved one to Living Branches.

The Health Care areas at Living Branches are designed and maintained to be safe. Hazards common in many homes have been adapted or eliminated. At the same time, our communities feel like home, not a hospital. Your loved one will experience all the comforts of home in a neighborhood that values growth, connection, and joy.

Delicious dining options

Good food is essential for health. Your loved one will enjoy delicious chef-prepared meals tailored to their dietary requirements. If needed, caregivers provide modified cutlery and help with eating.

Well-kept accommodations

Each resident makes their residence their own, and we’re happy to keep it clean and comfortable. Private and companion rooms are available, and all maintenance is included.

Family is always welcome

In our Health Care communities, residents can come and go with their loved ones. Enjoy family reunions, birthdays, or a special meal out on the town. Loved ones are also welcome to drop in and visit anytime.

Find care for the body and mind

Life doesn’t come to a halt in Health Care. When your loved one isn’t working with an on-site therapist, eating a meal, or visiting with family, they’ll have many chances to learn, make friends, and take part in on-site activities.

Always something to do

Both of our campuses offer numerous ways to connect with others. Early on, your loved one will get to know their community’s life enrichment coordinator. This person leads numerous activities, from movie nights to trivia to music. Residents are also invited to participate in special campus events with other residents across the Living Branches community.

Creative arts therapy

Music and movement can improve not just mental health, but physical health as well. That’s why Health Care residents have access to group and one-to-one sessions in art, music, and movement. In a creative arts therapy, residents are invited to express themselves and connect with others. No music experience required!

Opportunities for spiritual practice

As a member of the Living Branches community, your loved one can take part in their campus’ spiritual life. Church services, hymn sings, Bible studies, and other activities are available. Our dedicated pastoral care team is a great place to turn for help and support.
